The professional relationship may be discontinued for actions including, but not limited to, repeated missed appointments, non-compliance with recommended treatment plans, or abusive, threatening, or violent behavior by a patient or their family member toward our staff or providers.
If your care is terminated by the practice, we will fulfill our legal and ethical obligations to prevent patient abandonment by doing the following:

- Emergency Coverage: We will continue to provide emergency medical care and necessary prescription refills for a transitional period, typically 30 days from the date of the notice, to allow you time to find a new provider.

While we encourage proactive engagement, persistent, excessive, or unreasonable utilization of services that prevents other patients from receiving timely care, or that falls outside the defined scope of a standard primary care plan, may result in a review of your membership.
We reserve the right to modify, restrict, or terminate membership agreements. Should a termination be necessary due to utilization patterns or a breakdown in the therapeutic relationship, the practice will provide formal written notice and will assist in a safe transition of care in accordance with applicable medical board and abandonment laws.
